First step is antibiotics perscribed by the dentist. Then you'd need a root canal and a refill for that filling you lost. If the cavity was too big to re-fill, then a crown would be wise. As for what I personaly could do for you as a dental hygienist, there are quite a number of desensitization agents that I could place on the tooth to help ease the pain while the antibiotics take effect.
